Permission to Post Scout Photos

Dear Parents/Guardians,

This form was prepared and provided to obtain either signed approval or denial, in order to post your Scouts photo(s) on the Pack-1218 (www.pack1218.org) website. Only photos/data obtained at/from a specified BSA, Troop, Pack, or Public event may be used, if approved as listed below.

If the utilization of your Scouts photo is prohibited, efforts will be made to blur, modify, cover the face of your scout, in order to preserve their anonymity within the photo, if your scout is pictured in a group environment.

Any photo of a Scout submitted directly (hand delivery/email) by the parent or guardian of that Scout, will be accepted as preauthorize for use on the website (www.pack1218.org). 

Instructions: Parent or Guardian Initials, signature, and date below are required.

__________Yes, I hereby release Cub Scout Pack-1218, the Boy Scouts of America, and the BSA Orange County Council, from any and all liability and/or legal, equitable claims of any kind, related to photos/data being published on www.pack1218.org website. I understand that scouts attendance/participation at any event, (public/private) may be identified by first name only.

Note: Please print-out, read, sign, date and deliver (hand deliver) to one of the following individuals: Webmaster, CubMaster, or Committee Chair.

Forms will be kept on-file during registerd Scouts participation with Pack-1218. Permission Form and changes are allowed at any time, by resubmitting a newly signed and dated form. The most current Permission Form (on file) will superceed any other previously submitted permission form. After receiving your signed and dated permission form, a signed reciept will be returned (hand delivered or by email).
